Senior accident investigator training

Serious accidents and incidents require a range of specialist structured techniques to make sure that the underlying causes are identified - factors that have the potential to cause further accidents elsewhere in the organisation. This accident investigation training workshop presents several root cause analysis techniques to be applied by delegates on a detailed case study.

Duration

The senior accident investigator training workshop is three days' duration. It can be extended if there are specific issues that need to be explored in more depth.

Accidents and incidents are (fortunately) rare events, which can mean that there are few opportunities for practising the skills. We can provide focused training and coaching sessions to help investigators refresh their skills and knowledge, or to help them through their first investigation. See our investigator coaching section, or contact us using the form on the right.

We also offer an introductory accident investigator training course for accident investigators as well as a root cause analysis training course for incident investigation techniques that can be applied to quality problems as well as safety events..

Who should attend?

The senior accident investigator training workshop has been designed for senior managers and safety professionals who may need to investigate serious accidents and incidents effectively.

What will you learn?

This workshop provides a refresher of the basic techniques needed by accident investigators and then uses a detailed case study for participants to practise advanced accident investigation techniques, including:
  • change analysis
  • barrier analysis
  • Five Whys
  • narrative chronologies
  • timelines
  • time-person grids
  • fishbone analysis
  • event and causal factor charting, and
  • writing an investigation report in plain English.
Accident investigation training materials
Delegates will receive an accident investigator handbook containing good practice, hints and tips that can be used in the workplace when carrying out an accident investigation.

An accident investigation report template can also be supplied (in Microsoft Word format),
tailored to suit your organisation.
