Root cause analysis

Root cause analysis comprises many standard tools and techniques to identify, understand and resolve any root causes of problems or incidents.

This is a one day workshop.

Who should attend?

This workshop has been developed for anyone whose role requires them to carry out investigations into quality failures or other manufacturing, process or maintenance problems.

What will you learn?

Specific tools and techniques that the training may include are:

Is / Is Not technique (to define and measure the scale of the problem)
Timelines
Process flow mapping techniques
Change analysis
5-Whys technique (with simple fault tree)
5W2H analysis
Control barrier analysis
Fishbone analysis
Brainstorming/brainwriting (to generate possible solutions)
Evaluation matrices (to prioritise solutions to adopt).

The training will be structured around a single case study that will allow delegates to apply the techniques. The case study can either be drawn from our library of examples or developed specifically for your organisation.

Delegates will receive an investigator handbook as part of the workshop to help them recall useful root cause analysis techniques when they carry out their first investigation.
